Required Vaccinations: All Students
Minnesota law* requires all students born after December 31, 1956, enrolled in a Minnesota college or university to be vaccinated against the following:
- Diphtheria/Tetanus—Td (dose within the past 10 years)
- Measles—rubeola, red measles (2 doses after age 12 months)
- Mumps—(2 doses after age 12 months)
- Rubella—German measles (2 doses after age 12 months)
*If you graduated from a Minnesota high school after 1996 or attended a Minnesota post-secondary school, you are exempt from the above requirements.
How to Complete Your Vaccination Requirement:
To avoid or remove registration holds, complete the online Student Immunization Form by the sixth week of the semester:
Please note that this is a one-time submission that all new students are asked to complete. If this is not your first semester at the University, you have likely already met this requirement and you do not need to complete it again.
If you are exempt, complete the printable Student Immunization Form by the sixth week of the semester.

Locating Your Records
Where to look for your vaccination records:
- Your personal or family medical records
- Your healthcare provider
- Your high school or previous college
The Minnesota Immunization Information Connection (MIIC) may be able to locate your records.
College Vaccine Requirements
Academic Health Sciences
- Fill out the AHS Immunization Form fully. AHS form must be signed at the bottom by the healthcare provider who reviewed and attested to vaccination information.
- Email AHS form to [email protected] (forms will only be accepted via email). *Medical records will not be accepted in place of a fully signed AHS form.*
- If you are non-compliant in one area and need to update a specific vaccination, a new form must be filled out with information specific to that immunization. Only the updated information needs to be updated on the new form.
College of Veterinary Medicine
- Fill out the Veterinary Immunization Form fully. The form must be signed at the bottom by the healthcare provider who reviewed and attested to vaccination information.
- Email the form to [email protected] (forms will only be accepted via email). *Medical records will not be accepted in place of a fully signed form.*
- If you are non-compliant in one area and need to update a specific vaccination, a new form must be filled out with information specific to that immunization. Only the updated information needs to be updated on the new form.
