Frequently Asked Questions
What do I need to bring to my appointment?
Please bring your insurance card, or a photocopy of both sides of your insurance card and the policy holder/subscriber's name and date of birth.
How much will my visit cost?
It depends on your health insurance and whether you pay the Student Services Fee.
A co-payment at the time of your visit may apply. Any remaining costs for your care may be billed to you at a later time. Amounts vary by insurance plan. Consult your insurance card, or contact your health insurance provider for details.
With some exceptions, the Student Services Fee (SSF) helps cover out-of-pocket costs of care at Boynton for students with active health insurance and who have paid the SSF.
Who can be seen at Boynton Health?
The majority of services are open to all University of Minnesota students, faculty and staff. The exceptions are Boynton Health’s Mental Health and Travel Clinic, which only serves students.

Does Boynton Health provide absence notes?
Boynton Health Does Not Provide Absence Notes
- If a student cannot attend class due to illness, they should contact their instructor immediately via email, phone, or in person. See the University’s Makeup Work for Legitimate Absences policy and the Self-Reporting of Medical Absence form if verification is requested.
How to Report a Single Absence - For absences related to routine illnesses (such as colds, flu, etc.) or injuries, students should contact faculty members/instructors as soon as possible regarding their situation.
When possible, this should be done before a class, exam, or other activity is missed.
Multiple Absences
An instructor may request verification of the medical absence under the Make-up Work for Legitimate Absence Policy. In these cases, the student should use the Self-Reporting of Medical Absence from Class form that can be electronically signed and sent to the instructor.
